If your employer gives you the DWC1 form (linked below), the answer is yes. That is the official start of the Workers’ Compensation process. The form asks basic questions such as your name, where the injury occurred, your body parts involved, your social security number, and other relevant information.

Then, the form is given to the insurance company, which ultimately starts the investigation and claims processes.
